How it Works

Our staff meets you in the comfort of your home, an office, or another place in the community. We offer support, assistance, education, and community referrals you and your family may need during your pregnancy, childbirth, postpartum and through the first year of your infant’s growth and development.

For example:

During your pregnancy we can help answer any questions and concerns you or your family may have about prenatal health, nutrition, stress management, breastfeeding, childbirth.

Once your baby is born, we are there to assist, support, and educate you about infant growth and development, safe sleep, car seat safety, feeding and nutrition, calming techniques, understanding your infants’ cues, and postpartum care.

When you enroll with CPIF-MIHP, you will receive individualized care from your assigned nurse and social worker. They will work with you, your doctor, your health plan, and other agencies to make sure you receive services that support you, your baby and your family’s health.

How to Enroll and Make Referrals

Individual referrals:

If you or someone you know is pregnant or has an infant who is already receiving Medicaid, you are all entitled to our no cost in-home services. Medical Care Providers:

CPIF Resource Solutions proactively seeks opportunities to collaborate with medical care providers to improve the quality of care for individuals and their families. If your medical practice assists Medicaid enrolled pregnant women and or infants, you are eligible to utilize our services.
